General Information – SWP/USDA Food Rebates Information
Title
Description
To provide reimbursement to program recipients that incurred and paid delivery charges during the period of October 1, 2021, through August 15, 2022. This reimbursement is based on the availability of current state administrative expense funds.
Disbursement of Manufacturer Rebates for food products purchased by SFAs through SWP during the period of July 2022 through June 2023.
Disbursement of USDA Cheese processing rebates for food products purchased by SFAs through SWP during the period of July 2022 through June 2023.

Disbursement of Manufacturer Rebates received at the State level for food products purchased by SFAs during the period of January 2019 through December 2019.
To reimburse School Food Authorities for value of USDA Cheese used to process pizza products which they purchased through SWP between October 2018 and August 2019. USDA Cheese for processing is purchased using SFAs entitlement.

SFAs are to use this form to report to the state any USDA Food they were allocated and ordered from the warehouse, but did not receive.
Rebate application Fillable Form for those NSLP sponsors who do not participate in the Statewide Procurement Program. Rebate applications must be filed monthly at the school district level.
